Dietary Difference Accountability Standard:
We provide all our reports in a facility-customized QAPI format that tracks all NAR categories, your facilities weight loss QMs, and siteable issues are provide to you and discussed with your IDT.
Food and Nutrition QAPI Monitoring is built into the Dietary Difference Documentation System and serves as the backbone of all our reporting to you. Our reporting and documentation formats were designed based on surveyor investigative protocols and are continuously updated as regulations evolve.
Each visit you will receive:
• Our consultant report details documentation of assessments and nutrition consults completed, including changes to diet and supplement orders, recommendations to medical team and updates to care plans. You will also be informed of changes to resident preferences and issues requiring attention from nursing or the FNS department.
• A copy of our Nutrition at Risk Weekly Weight Tracking tool, which can be used in NAR or IDT meetings.
Each month you will receive:
• Our monthly consultant report provides statistics on significant weight change, and allows you to track documentation on pressure areas and dialysis
• Our comprehensive monthly weight report gives a six-month weight history and highlights significant weight changes at a glance for your entire resident population. This report is designed to be shared with your medical team to keep them informed.
• Our significant weight change report with weight change rationale and interventions in place
• A detailed breakdown of interventions in place to address pressure areas
• Our Kitchen Sanitation Audit is conducted monthly and provides a comprehensive proof of QAPI that is reviewed with Food and Nutrition Services at risk meetings.
Dietary Difference Standard for Food and Nutrition Services:
Whether your Director of Food and Nutrition Services is a facility employee or contractor, your Dietary Difference® consultant is the license-holder under which your FNS department operates. As a result, your consultant is accountable for FNS departmental oversight, and will be actively involved as much as needed in the areas of:
• Menu development
• Review credentialing of chef or Dietary Manager with recommendations and support options
• QAPI monitored survey readiness
• Tray card accuracy/diet order agreement systems and periodic audits
• Staff training based on ServSafe® models
• Promoting a home-like “Service First” environment
• Cost reduction/comprehensive PPD support
You can elect to use our full library of Food and Nutrition Services Policies and Procedures and forms, which adhere to state and local guidelines, incorporate surveyor investigative protocols and may be customized to meet the needs of your facility.
